Today we have a review of our patient Dhanesh Tiwari with his hair transplant journey in this article with photographs and hope you will enjoy and learn from this.
Dhanesh Tiwari is a 29-year-old male from the Kolbad area, which is located in the City of Lakes, Thane, in the Maharashtra state of India.
He is suffering from a Grade of baldness level 3 to 4V. He had started hair fall complaints since 6 to 7 years ago. He had started noticing baldness since last 3 to 4 years ago.
At present, he has lost his hairline completely with hair loss in front area. But his middle is in good condition with mild thinning. But the crown area has more thinning of hair requiring refilling hair transplant.
Along with hair loss, he has also complained of mild dandruff. Patient had mild pimples and no active ongoing hair fall.
In family, his father has baldness. So, genetically he is more prone to hair loss.
Dhanesh is not suffering from any medical issues such as high blood pressure, diabetes, asthma, or any other disease at present.
Previously Dhanesh had not undergone any major surgery in the body.
Patient has not any allergy to food or medicine.
He has no any history of keloid in the body.
Dhanesh had taken some treatments for hair fall such as minoxidil, and multivitamins. Such treatment showed mild improvement initially. But later on, no major improvement was noticed.
Finally, he consulted Dr. Jinkal Kunjadiya, a famous dermatologist at our Surat branch. He was consulted with all details about hair transplant, and his hair health conditions. And also discussed his expectations and results of hair transplants with all advantages and disadvantages.
After a thorough understanding of hair transplant surgery, Dhanesh got ready for the surgery at the Surat branch of Hairfree and Hairgrow clinic. And finally, he was taken for a hair transplant procedure.
Dhanesh Tiwari’s hair transplant was done on 24th July 2023 using Bio-FUE technique and after transplant 5 PRP was done at 2-month intervals in 1 year.
He was also treated with supplements, a DHT blocker, and Minoxidil 5% and 10% solution post-transplant to maintain and grow his existing as well as transplanted hairs.
Proper planning and hairline design were done by Dr. Jinkal Kunjadiya as per patient’s desire and the Hair loss condition of the patient. The front area with the temple was marked. Crown area was also marked so that we can refill some grafts in that area.
Also, a Trichoscopy analysis was done to check donor area hair quality and existing hair quality in the recipient area. The patient’s donor area is average to good hair quality is thick and type of hair is mild wavy to straight and black hair.
Then after the anesthesia skin sensitivity test was done and found non-allergic to anesthesia content.
Afterward, the Patient’s head shaved for the procedure and cleaned with betadine & alcohol
Now Dhanesh was taken to the operation theater for a hair transplant and local modified block anesthesia was given with a vibration painless technique.
Slit making was done by an expert hair transplant surgeon team with Dr. Jinkal using a Metal Saphhire tip blade and the size of blade is 1.2 for hairline and 1.4 for rest areas using magnifying loupe.
Making a slit that is a small incision in the recipient area is a very important step because the angle and the direction of the slit give proper direction and overall look of the future transplanted hair.
Extraction of grafts is carefully done by an expert doctor using a magnifying loupe and 0.8 mm sharp serrated sterile titanium punch used for extraction.
Around 140 to 160 minutes time taken for exctracting the grafts.
If we classify the Graft analysis of Dhanesh, it was thick hair, light to dark black in color. Double-rooted grafts are more in quantity. 20% were single-rooted grafts and the remaining 80% were multiple-root grafts containing double and triple-rooted grafts from all extracted grafts.
Dhanesh has also an average number of 3 rooted grafts. So, he will get more benefits in terms of density. A total of 4040 grafts were extracted.
The extracted grafts were stored at 4* degree temperature in a graft chiller machine dip in RL solution.
The final step is the implantation of extracted grafted into the slit made. The Graft insertion was done by using a K.E.E.P implanter of various sizes for 1,2,3 FU grafts. The approximate Time taken for implantation was 3 hours.
After completion of implantation dressing was done, post-procedure medicines were given and all precautions were explained in detail. A personal follow-up doctor’s number was given, so that proper follow-up can be taken, and if there is any query, a patient can directly call the doctor.
After the hair transplant procedure first 3 months patient was in the shedding phase and by this time around 60% implanted hair fell out. All the hairs didn’t shed at a time.
During this phase 2 PRP were given at 2 months intervals. Exiting hair in the middle and crown area has shown a good response to PRP & medical treatment. And his hair fall of existing hairs stopped.
Donor area skin heals completely by the end of 30 days & hair density looks normal by the end of 2-3 months in the donor.
This phase starts after 3-4 months. At this phase, implanted hairs started coming out of the scalp slowly. 3rd PRP was given to boost transplanted hair growth and existing hair. Trimmed existing hairs almost achieved full density look at the end of 5th month.
This phase starts at around 6-7 months. During these months implanted hairs grew fully achieving nearly 80% of transplanted hairs, though length of each hair is not similar, it is enough to do a haircut and Dhanesh was fully satisfied with it. 1 booster PRP was given at this phase at 9 months.
Full hair transplant result of Dhanesh Tiwari was achieved at 10 months. You can see the pictures below. Here patient is able to do any hairstyle. Patient had 2-3 haircuts of the donor area as well as transplanted hairs.
No major medical care is given now, a patient asked to continue minoxidil & if required do booster PRP as needed in future for maintaining existing hair.
